We are so glad to welcome you to our office. Below are 2 forms to print, complete and bring with you to your 1st visit. If you’d like you can send them prior to your visit via: fax 520.881.3759 or email: tucsonsmileteam@gmail.com is also appreciated. Please arrive 10 minutes prior to your reserved time with your insurance card (if applicable) and picture ID.
Contracted Insurances:
Aetna, Ameritas, Assurant, Blue Cross Blue Shield, Cigna-Radius Only, Delta Dental, Guardian, Metlife, Principal Financial and United Concordia. As a courtesy we are happy to accept & submit any PPO insurances not listed above. PPO means that you are allowed to see any dentist. We understand the value of having dental insurance and it’s important to note that the MOST important factor when choosing a dental office is to make sure you are comfortable and trust your dentist and their team. Insurance doesn’t care about you but WE DO. We invite you to call or come visit our office to see what a dental team should be like.

Once on Beverly, the 1st street on the right hand side is Knight Drive and the 1st parking lot on the right hand side is where you’ll find us. Upon your arrival a smile will be waiting for you.
